I remember that really well. It was a leaked script for Halo 3 that came out in 2005. It picked up right where Halo 2 ended, with the Arbiter, Keyes, and Johnson on Delta Halo/High Charity, and Chief on Truth's ship and on Earth.

It actually did a much better job tying up loose ends for the series and had better continuity than the Halo 3 we got in the end (ie: The Ark was Earth). It was so detailed that it even had walkthroughs with the goals for each level. It was so well done that I'm convinced it was legit. The only question was whether it was just one of several options that Bungie chose from, or if it was the one true script that then had to be scrapped.
