Maybe not pages of notes. Depends on how well the doctors can condense the medical information for a quick-start guide. What exactly is need to know?
This would probably work better as an unremovable medical ID bracelet.

Better yet, maybe you could store the medical records online, accessible by scanning a QR code on his chest?

I'm no expert, but I think it's easier if death is a long time in coming. Maybe declining over five years is less tragic than being struck by a heart attack out of nowhere.
