Click the 3-bar button in the top left

go to settings

click show advanced settings

Under the privacy setting section click "clear browsing data"

untick the stuff you want to keep, but make sure to tick the autofill form data box.

And before you ask, yes, this will clear out anything else you have set to autofill. Tis the only way.
