Reach Infection Achievements

So I've jumped into Reach for a little bit to try and get some of the achievements I missed from the Anniversary and Defiant sets. I've done both of the Infection ones at least twice, both in vanilla Infection and other variants, and have not had either the Emergency Room or All Alone achievements pop. Are these glitched? Do you have to be the last man standing for each round or something? I've definitely gotten 3 infections in a single round; do you have to do it without dying? I've done that at least once if not more.

Anyone want to boost these this week?
