GURPS says that traditionally the Dwarves were ruled by the Ephors; there's no mention of a King above them. I think the King appeared post-TFL (after all, the only king mentioned lives during the Myth2 era) after the city-states had basically been nullified and all the surviving Dwarves formed a single state.
The Stoneheim Dwarves collapsed the barbican because they knew they were doomed if Myrgard had fallen and could no longer fight with them. Again, they were close allies of each other, and they had a common culture which made them both enemies of the Ghôls and of Balor; they just weren't politically bound together under a single ruling government. I don't think.
