We are told that all shades were once avatars and as yet, in the Myth world we have yet to meet an evil avatar. As for Fallen Lords, we know at least some of them were once good, e.g. Damas/SB, Connacht/Balor, and Myrdred/The Deceiver. The histories of Ravanna/Shiver and Bahl'al/The Watcher are more clouded, since we don't know much about their pasts.
You could argue that Fallen Lords need not have been good once, referencing the fact that Shiver is referred to as "an ancient evil". Whether this refers to Shiver rather than Ravanna is unclear.
Long way of saying, "unknown", but it's my opinion that, given the cycle of things, all Fallen Lords were once good. I have no compelling evidence to prove this, just intuition based on my understanding of the Myth world's yin and yang.
On the other hand, if The Watcher is really a goat, maybe he just went straight from dumb farm animal to Fallen Lord. :) Others will no doubt have more enlightening words on this subject...
