There's one other case of it--when Alric had arrows made out of the Watcher's own arm, the narrator said having a piece of the Watcher was "like knowing his true name, only better."
Thing is, "Ravanna" doesn't just show up as Shiver's name in Myth2. GURPS lists Ravanna, the Voiceless One, as the sixth Fallen Lord (Soulblighter, The Watcher, The Deceiver, Shiver, The Lurker, The Voiceless One), separate from Shiver. So all our theories are trying to figure out why Shiver and Ravanna were generally treated as different people, but then identified as the same person by Myrdred in Myth II.
