I'll probably get blasted for making some mistake, but I believe that Moagim used the mykridia as you mentioned and the only other Fallen Lord we hear of is Tireces, (Spelling?) but we don't know what sort of armies he used. Those are the only two Fallen Lords we know of besides Balor. Keep in mind that this all spans a few millenniums and history does tend to get lost in time, or that could have been the beggining of the cycle, but Im not sure.
