Well, it seems to me that the Fetch are able to somehow keep the skins they wear alive, so that the skin can take an endless amount of damage as long as a Jman's around to heal it (which causes the skin to regenerate and patch up its holes). (Supporting evidence for this--that Fetch skins are "louse-infested"; lice don't live on dead creatures) So they probably don't need to stitch anything, or find intact whole skins; they can just slap on a "patch" of intact skin from one of their mostly-fried victims and have it auto-graft to the surrounding skin.
As for how they got here in the first place--it could have taken a while for Wyrd to first "notice" their presence, or Balor could have somehow shielded them from his view for a brief time, or sent the skins to the Fetch world or an intermediate plane first and had the Fetch "get dressed" before coming to Earth--or yes, he could have rebuilt the interior of human sacrifices into Fetch, or teleported out their inside bits and teleported the Fetch in. Still, GURPS and manual text both agree that the Fetch now wear the skins of their *victims*, so they must be adept at patching and replacing them by now.
