The one whose body was destroyed by fire could still have been "beheaded" in a sense, and he is the only questionable one. I don't think that the Leveller's head can be destroyed by fire, So I picture it as the rest of the body burning away, leaving the head... hence the emphesis on BODY was destroyed by fire, not just saying "He was destroyed by fire"
If Alric with all his magic couldn't destroy a leveller's (Balor's) head, something as simple as fire CERTAINLY can't destroy a leveller's head. So really, you HAVE to picture it as above... Body melts away, talking head left jabbering away in the ashes.

Of course, additional evidence here would be that Connacht wanted the Head to be underneath his empire, so he could contain and control the Head, so its power could never grow... if he puts the Head somewhere else, it could grow in power and escape, perhaps. Again - more circumstancial evidense.
Alric couldn't destroy Balor's head, Fire can't destroy a Leveller's head, Connacht couldn't destroy Moagim's head. His best bet was to seal it in the catacombs beneathe his city or bury it beneath his city so that the might of the Cath Bruig empire could control the weakened but still dangerous Head.
