Since Dave answered your other questions, I'll grab the Codex one.
Yes, the Myth TFL narrator resolved to destroy the Codex *if* it fell into the hands of the Dark. But it didn't. You guys got the Codex to the Nine successfully. After the war was over, Alric must have sent the Codex back to Covenant for safekeeping, where it hung out in the library until Myth 2.
