I don't think the Total Codex works like a regular book or Encyclopdeia as you're putting it. You can't just look up the name of anyone and see their fate. I think the book tells you what it wants to tell you. The guy in the story just opened to a random page and read about The Summoner ressurecting the Myrkridia. What are the odds of that if the book contained the fate of EVERYTHING. In my opinion, you'd need a pretty damn big book to contain the info on everyone and everything and what will happen to them. As mentioned before, the Codex probably just tells you what it wants to or maybe what you want or don't want to hear. How the Codex works is another completely unsolved Mystery of the story.
