Nyx created the Trow; beyond that it's anybody's guess who created anything else. Wyrd is the current supreme monotheistic deity, but the only solid data we have on him (that is, something not in GURPS) is that he wouldn't like the fetch if he saw them naked (I'm probably missing something--I've heard he's supposedly father of the Head, but I've never really bought that).
The leveller isn't related to anybody, it isn't even an individual. It's a "transient deity", that is, a spirit which invests one individual every thousand years with evil power. Balor is the classic example. The Leveller changes identity every thousand years so it can't really be related to anyone, nor is it a god in the classic sense. Sorry.
