Two different incarnations. Keep in mind Soulblighter also has two proper names. (Damas and Anshar.)

I don't think anyone ever saw the Voiceless One. Her GURPS entry certainly didn't make it sound that way. I think Shiver and the Voiceless one weren't two different names for the same person, but rather two different aspects. Both Shiver and her previous incarnation inhabited the same body.

Those spells were apparently part of the Voiceless one. Perhaps there's some aspect that doesn't work without her. Your theory requires the existence of seven Fallen Lords apart from Balor, which contradicts every single source.
