Here's what I think, though it's not my own theory (someone else - I thought you, Dan - posted it earlier).
Ravanna, The Voiceless One, was the old incarnation of Shiver. She was killed, sent into some kind of "eternal sleep". She was reborn as Culwyeh, who grew up to become Shiver. Balor, in recruiting his Fallen Lords, awoke Shiver's old incarnation, the Voiceless One, while Shiver was still alive. Her spirit was forced back into control of Ravanna, while it still controlled Culwyeh, creating the unique situation of two bodies under one spirit at the same time. Perhaps they reflected opposites of her personality; Culwyeh's bold rashness versus Ravanna's stealthy sneakyness.
Also, GURPS tells why Ravanna is so pissy in Myth II. Since her weakness is vanity, and the corporeal form of her true spirit is so damned ugly, she is driven into a continual, maddening rage. She was trying to restrain herself when talking to Myrdred, but when he called her Ravanna she cut out the funny stuff and hurled everything she had at him, then ran away.
