It sounds to me like it's just a glitch in the text file... er, a typo... and that they're completely different people (?) who ended up having the same name. Probably the GURPS writers made this mistake for the same reason this topic has been going back and forth so long--Bungie wasn't at all clear on this stuff and gave us almost nothing to work with.
Which makes for an interesting point: in "Shiver" (M2), the Deceiver, in his infamous sound file, says "The years have not been kind to you, Ravanna..." Maybe advanced age and the negative effects of losing a dream duel to Rabican are what made Shiver so nasty--maybe at the start of the Great War she was something to see. We know vanity is her weakness--surely with all her magic she would have found a kind of "glamor dream" (glamor in the Celtic sense) if she could. Okay, before this gets too sexist, I'll contradict myself and suggest an alternate theory for the similar name:
Shiver and the Faceless One are both old school, right? Maybe "Ravanna" was a common name back then, like "Jennifer" is now. I know it sounds ludicrous--but if you had two powerful sorcerers born in the mid seventies and they both grew up to be fallen and they were both named Jennifer, you'd have the same problem. :)
