One thing that the book did talk about was the Voiceless One slipping through the night on missions. Named Ravanna, the loveless child of the unwed dawn. This may well be a Mahir name. So here's a theory:
Culwyah dies, somehow is turned into a Mahir by some strange occurance which really can't be explained anyways.
Someone, most likely Soulblighter, decides that he could use a powerful sorcerer on his side, and resurrects Culwyah using Tramist's (Trabist's) Mirror. The spirit of Culwyah is still wandering around, unknown to everyone else. This reincarnation of Culwyah is called Shiver. The spirit, which is eventually captured by Balor, is named Ravanna so that Shiver will never try to reunite herself with her spirit. The spirit can't talk (and can't tell Culwyah about her true self), but is pist at Balor and decides she won't do what Balor wants, and goes off at night (when she cannot be seen) and kills a few thousand soldiers (or ghols, or whoever is pissing her off at the moment).
Interestingly enough, she is referred to as "the mother of plagues and first lady of [the] Fallen Lords". Was she somehow the lover of Balor? eeeeeeeew.
