Actually, Moagim defeated the hero, but got jumped later. Killing him didn't stop it from being a dark age though.

I'm guessing he put all the artifacts he was supposed to get rid of in his secret temple. This would also explain how he got the Tain.

Oddly enough, it never even mentions his death at the end of Shiver.

Not many times. She lost her third life in Myth 2. The way GURPS explains it, the Voiceless one was the first incarnation. He/she died a long time ago. Shiver was a re-incarnation. However, Balor awakened the Voiceless One from eternal sleep, so both co-existed. Possibly in the same body; it isn't clear. It mentions she lost most of her magical abilities in Myth 2, including the whisper dream. It's unlikely she was able to cheat death.

Maybe not. Take a look at page 3 of Tales from Myth. The Fallen Lord calling the ghôls is not one of the known ones. He wears a horned helmet, but it is not like the Watcher's or Balor's. His face is in shadow. It could theoretically be the Lurker, but that's very masculine helmet.
