We have a number of names that are not conclusively assigned to any of the four known Fallen Lords, but none of them are from the games: The Voiceless One, The Faceless Man, and Bonesplitter are from prerelease materials, and Bheil and a bunch of other confusing misinformation is from GURPS. I no longer consider GURPS canon because of all its obvious contradictions of in-game materials, and pre-GURPS the best candidate for assigning one of those three names to the last of the four known Fallen (Shiver) was "Bonesplitter"; it's not mentioned on the Fallen Names article, but her vanity also makes her a great candidate for a former Myrmidon, and Bonesplitter is an archetypically Myrmidon name. Myth 3 also depicted her as a Myrmidon, for what little that's worth.
So that would leave the two unknown Fallen Lords as most likely named The Faceless Man and The Voiceless One, much befitting their silent invisibility in the story.

No canon information; all we know of the Twelve Duns is that it's "closer to the Trow demesne than any sane man has dared in two generations", i.e. since the Great War. So, probably not the Berserks' homeland.

Don't think anybody's suggested the Leveller from BEFORE Tireces. Plenty of people were suggesting Moagim, which is the theory I finally came around to too.

A "mantle" is a figurative expression for a title, position, or responsibility, deriving its use from a literal article of clothing (a broad-shouldered cloak) that signified a position of authority like that of a king. So for example, if I were to retire from admining this forum, someone else could take up the mantle of adminship; but that doesn't mean I'd have to ship them the cape I wear whenever I'm acting as admin, I'd just have to set their account to Admin. So Balor saying "wore the mantle of the Leveller" is a poetic way of saying he held the position of being the Leveller.
