It's been a while since I've played with it (end of 2004), but I seem to recall it being pretty brutal in the realism stakes. There were ways for better (veteran) characters to avoid the worst of damage but they'd still pick up some nasty injuries on a critical hit or somesuch. I didn't have a single death during testing, but I had a fair few character re-defining injuries.
