With respect to information about the Head, the journal writer is very common indeed. He wasn't anywhere near Madrigal through most of the war, and all information he had about the Head was third- or fourth-hand. He wouldn't know anything special about the Head that the intellectual community as a whole (the folks that wrote the manual, I mean), wouldn't know.
I'm pretty sure the Head got more than just two of the Nine killed. Chances are it knew they'd get trapped in the Tain, which got two other Avatara killed, and from the narrator's statement that "the Head had been double-crossing them since..." I'd infer that it caused multiple other defeats of the Nine as well.
