The Deceiver's and his forces were defeated at the Stair of Grief. Myrdred himself was trapped in ice 'til Alric sent people to free him some 60 years later.
That kinds puts him out of luck when it comes to destroying the cities in the West.
Which Fallen Lords were mucking about in the West at the end of the Great War, then? This would leave perhaps Soulblighter and one or two other Fallen Lords to raze the West. Personally, I would think Soulblighter was kept in the East, while the other two flattened the West.
As for the Head, I think that when the rest of the Nine "had it out" with it, it meant they killed that Head for good. Whatever it could muster in terms of defences was most likely troops it could persuade. I doubt it was capable of magic other than keeping itself alive.
