The Faceless Man and BoneSplitter come to mind. As to why they never show up, maybe it's as simple as the player never encountering them. You should be thankful you survived within 1000 yards of SoulBlighter for so long, do you really want to deal with more Fallen? So we never see them in TFL out of mere chance, they're probably dead by Myth 2, and Myth 3 doesn't count.
