One of the shades is described as enduring "the indescribable agonies of the Fallen." From this I would infer that all those sustained or physically enhanced by black magic--undead and Myrmidons and Shades and possibly even warlocks--are considered Fallen. A Fallen Lord is just an especially powerful member of this group. Most serious threats to the Light, even in the middle of a Light-ruled age, would be Fallen; villains just don't get very far without magical backing.
