I don't know why, but I was always under the impression that Mazzarin was still alive near the start of TFL story. The Watcher killed him with seven waves of Thrall. Take a quote from Crow's Bridge narration. "None of the people understand yet what is happening, and they are frightened" and "Did you hear what happened at Avon's Grove? There were no bodies. Yeah, what did they do with all the bodies?"
These people had never seen thrall, wights or soulless before. BOOM "What the hell was that?" It follows that The Watcher found the dream of unlife recently, and killed Mazzarin shortly thereafter.
