I realize this may seem a bit far-fetched. I'm not even sure I believe it myself. What if these lists overlap? It would certainly explain the evidence for both Shiver=Bonesplitter and Shiver=the Voiceless One. It would also mean that the Faceless Man=Soulblighter.
Of course, for this to work, it would have to be possible to simultaneously be an ancient evil and turned from the light. This could mean they were turned from the light a long time ago. This seems to be the case with Soulblighter. At least, he turned from being associated with the Light, which could be considered the same thing. Shiver could have been the one who led her whole tribe/clan to Balor, having turned from Berserk ways significantly before the rest of them in order to pursue magic.
As for the name Ashfear, perhaps Balor originally took a Myrmidon name when he created the Myrmidons. Possibly, he was the first one.
