I think the Head was in the catacombs below the city. Truan alone could not move all those tons and he was the sole survivor of the expedition that went to get it, so it seems logical that The Head was "buried beneath tons of sand and rock" in a chamber below the city.

Upcoming CoD levels "Shadows of the Sword Age" and "Whispers of the Wind Age" will tell the story of the expedition into the catacombs and Truan's flight from the city.
