I know, I know; I'm posting theories in response to my own question, I really must be some kinda loser... But people didn't respond with theories that we could argue about! So I'm going to post mine, and if you like, CHEW them like your CUD!
1) As these are rather interrelated topics, I'll attack all of them in one fell swoop: The leveller, most likely, has something to do with dreams; I'm not sure who, but a some months ago, somebody posted something about dreams that developed into a rather extended thread, and he (or one of his responders) had a rather interesting theory: that dreams will eventually corrupt their user, in the abscence of all of them (and no mortal being has ever learned all of the dreams that make up the One Dream, right?) So the leveller, accepting this theory, could be something of a multiply aware divinity: it targets those most proficient in these dreams to become its prime agent, but spreads itself out amongst others less proficient (example: Balor and the Fallen). Perhaps this is how Balor magically binds the Fallen to him; since he is the primary agent of the leveller, the rest are bound to him by the influence of that power within him. And to sort of tie in the cycle of levellers (A-spirit and B-spirit), one could think of it thus: the first time that the leveller corrupted the A-spirit, there were no other truly proficient avataras extant, thus forcing the leveller to put all its power into a singular being; however, when it first corrupted the B-spirit, there were others that it could spread its power amongst... This sort of suggests that the leveller and Wyrd are connected through the One Dream of Wyrd (perhaps it was the leveller who broke it, or perhaps it started to exist when the Dream was broken and thus made corrupt), and would also suggest that Wyrd is the only god that had a "One Dream"-type of thing.
