Pls read the Myth Comic. And you see it is illogical as we see Shiver as a little pile of dust. Also it says (I think in a flavor text) that SB gathered up the soul of Shiver with the Tain and made it corporeal. So he didn;t need a body and the comic also states that Shiver's spirit was cast into the Ether, so he was able to use the mirror.

One:The city could be anywhere, there is zero proof that it is Madrigal. It could be Muirthemne, or more likely Covenant since we know that it was SB who sacked it.
Two:That picture was a Fan Art that Bungie put on later prints of Myth II.
