I just assumed the apple was supposed to bring to mind that other great deceiver, the serpent in Eden. As for the eyepatch, here's two suggestions--
1. When the zerk said the resurrection spell, part of the healing effect restored Myrdred's eye. Who knows, maybe the way he said it wrong actually increased its power.
2. As the Deceiver warped everyone away after his resurrection, he actually left and substituted some poor mind-controlled sap who had the bad luck to look a lot like him (plus an eye). It's this puppet who you have in your party for the rest of the game, and the Deceiver channels his magic through him from somewhere far away. This would explain why the Deceiver's killed so easily by Shiver; his work of vengeance done, he has no more need for his puppet and it's probably convenient to have Alric think him dead.
