IIRC, Mazzarin was killed during the Wind Age, when Connacht ruled and had not yet become Balor. So after Moagim had been destroyed, The Watcher attacked with thrall and managed to kill Mazzarin before Connacht imprisoned him. (This parallels the events of Myth II; after the Leveller dies, a lesser Fallen Lord tries to conquer the world and does a lot of damage but eventually fails.) We don't know whether The Watcher discovered the Dream of Unlife before or after Connacht destroyed Moagim (although I'd suspect afterwards because Connacht/Balor hates Myrkridia, which we know Moagim *did* use, but doesn't seem to mind the undead), but we do know it was long before the Great War in TFL.

The Soulless are referred to as "one of Soulblighter's most successful experiments with reanimation," so apparently they're his idea. The Deceiver doesn't usually make creatures; he just charms whatever creatures are already nearby. Possibly the tag name you mentioned was for a Soulless with special coloring or behavior who happened to be assigned to The Deceiver's army--that is, "soulless (deceivers first one)" is just the first Soulless belonging to The Deceiver that you see in the level.
