Keep in mind that you never really see the city of Madrigal. You only see the port area where the ship is waiting at the dock. Alric escaped through a back entrance and was more than likely a ways from the city when you first gain control over him. So I would highly doubt that what you think is Madrigal is nothing more than a small port town.
