Balor was not a Fallen Lord, he wore the mantle of the Leveller. Under him were 6 Fallen Lords: Soulblighter, Shiver, Deceiver, Watcher, and 2 others that were unnamed in the game itself.

So in fact Balor was not a Fallen Lord, but the Leveller. Hope that clears things up for you.
