Hmm. In M2, Madrigal is capital, where King ALric lives. In M1, he is born as part of Royal Family and lives in Covenant. In M2, Covenant has been rebuilt, and in the comic it calls the leigions (not THE Leigion) "newly formed", and is set before sack of Covenant, as Mauriac, who evacuated the Royal Family from Covenant, achieves his fame at the ned by killing Fangrinder, and says "new alliances were forged" "as the War spread ever Westwards", so the treaty with the fir'Bolg must only have been one of them. M3 says "Western lands of the Provinces", and it's also said that the Free Cities of the North were founded during the Axe Age by nomads/settlers.
Thus, I think that they were city-states, and maybe by the time just before Balor appeared larger cities like Madrigal and Covenant may have had smaller towns under their influeces, but when the War moved into the West the City-states and petty-kingdoms united, maybe after sack of Covenant, and after the War, seeing as Madrigal was the site of the first major defeat of the Dark, and indeed the turning-point of the War, ALric situated his throne their. I'm guessing he was King instead of other royals because: (a) His brilliance won the War, and, (b) All other royal people were almost certainly dead by the time of Balor's defeat. :(
