From the wierd lucid surrealness of the comic, I'd say that Dream Duels were a battle of minds in a kind of "dream world": this isn't saying that you had to go to sleep though, though on second thoughts maybe a trance-like state, as many ancient cultures believed that when you entered a trance you were on a spiritual plane of existance, as the blood flow to the part of your brain that defines physical reality is cut off.
