First of all, I think that the "20 years war" is a war against the forces of Balor, after all, theres the "Great War" and many smaller wars in it.

"The province" dindt exist till the end of the great war. Before all the great cities of the west were individual city-states with thier own king. After he defeated Balor, Alric became king of what remained of civilized lands, which included all the cities of the West which came to be known as "the province".
