Seems to me that the only Fallen struck personally during the war was Shiver (believed to be killed) and the Watcher. Any others would have probably been mentioned too, because, hey, that's a Big event for the Light, regardless where you are, it's just something that would become well known very quickly.
Sure there were other battles not spoken about, but I don't think any Fallen were killed during them.

Myrdred: tricked by Shiver and blown up real good

? [Lurker, Bonesplitter?] and ? [Faceless Man?]: presumably hunted down after the Great War, as it says in the M2 manual.
Besides, in the M2 epilogue, it says that the fallen lords are dead. SB was the last one I guess.
