Here are few other unmentioned things:
On "The Ibis Crown", when you enter the second room with stygian knights and taunt with your berserks, they will all attack you at once.
On "A Murder of Crows", there is a unused sound by Deceiver "Why release me now, useless beasts, you have killed us all!". According to the script, he is supposed to say this if you release him after Soulblighter's been warned. But instead, he just proceed in normal way, but doesn't pay attention to Soulblighter, and the level won't end.
