Back in the day I actually took the trouble of mirroring myther.com! Good thing I did. Below is a plain-text rendition of the easter-eggs page, well, everything that hasn't been covered already to my knowledge, and I've summarised it a bit. I've also included the names of the original guys credited with finding these...
Fishing in the Lava - In the Forge, there's a guy in the very top right-hand corner fishing.
Easter Island head on The Wall.
The Relic - "there is a cool face that is visible in the overhead map. It's located fairly close to the bottom between the two rivers on the map. It may not be obvious at first ... but its definitely there. Its rotated about 55 degrees counterclockwise if that helps - (Stirring)". (I don't think this is talking about the volcano skull face, but something else. Also don't forget Shiver under the bridge in this level - Iron).
Friends Like These - "shoot the Deceiver with your archers by control clicking the ground near him. Soon he will say 'I dont think we should be doing this'. Keep hitting him. Eventually you'll get rushed by the 3 trow and a lot more of their buddies. Kill the 3 trow and you win the level. (Damien Warr and Art Lamarche)"
Salvation - "Sneak your men around to the north of the map and you'll find the main hut where the majority of Brigands are standing. You'll hear them say "So, what are they going to do with all the bodies?". The other guard then responds "Who cares ... Stop asking questions! (Traynn and TheRabbit)".
Broken Path - "They killed Rurik! You bastards!" A clear parallel with South Park.
The Relic - the hidden Griffin.
Back on The Wall - the Bungie song coming from the hut in the south.
The Baron - "In the pregame pictures, look for the evil baron sitting at his desk writing. On the bookshelves behind him are a copy of Jaws, and another of Star Wars. (Mark Bassett)"
Beyond the Cloudspine - "The braille DAVID". This was spotted by someone called Christopher D. Goldman (sorry Smasher).
Shiver - There is a woman's face in the top right corner of the map which you can see if you look in loathing's overhead view (Ben Lachman).
Ibis Crown - it says "Ramirez", and "Repent, the end is near" on the far right wall (Entrail).
Ok, that's it. I haven't checked any of these, though.
