The mirror doesn't require that the recipient's body be intact. It just grabs a soul out of the ether and makes it corporeal. Shiver used to be a beautiful maiden (see postgame loss screen from "A Traitor's Grave"), but her soul is ugly, so now the corporeal form of her soul is ugly as well. So just because Balor's head is falling down a bottomless pit doesn't mean that his soul can't be grabbed off the ether (though he would probably be Connacht again then, since The Leveler has left him). Unless, of course, Balor's soul is still bound in the physical world by his still-living head. Can he leave his head if he wants? Or is he trapped in it, forever falling down the Great Devoid?

: Deciever. so, it he actually dead, or does he just
: want phelot to give him a trip through the magic

I think the mirror is still back in Soulblighter's temple, along with other scary relics like his Tain shard.
