confuzzling Leveler question

The more I read about the Leveler, the more I get confused. How many Leveler's were there (up until Balor, the latest)? And what are the names of the ones we DEFINETLY know about? That obviously does not include pseudo-myth (like GURPS which has numerous incorrect facts) and the background story for some joe schmo's plugin.

Especially confusing is "Moagim Reborn". Just exactly How Many leveler incarnations did Connacht kill?
