Most likely it works the same way as in the Earthsea books; you've got a public name and a secret, true name, that no one except you and very close friends and relatives know. "Damas" and "Myrdred" and all those aren't true names--they're just the public names of the Fallen from back when they were good. Chances are we've never been told the true name of anyone in Myth.

I didn't interpret it as the Watcher stomping alone, but rather he and his enormous army of undead all marching together. That would certainly shake the ground.

Not all Fallen Lords can float--in fact, Shiver's the only one we've seen do it, IIRC. And she doesn't seem to be any better at it than Shades. Whatever force they use to repel the earth with must fall off quickly with distance, so that they only have strength enough to support themselves a foot or so above the ground. The S'pht seem to work the same way.
