I've noted that only 5 names are given to the 9, leaving 4 unknown, and only 4 of the 6 fallen lords are named as well.

Perhaps this has been discussed previously, though I'd found nothing regarding it in my searches.
Firstly, would Balor himself be considered one of the (5th) fallen? Or would his leveler status higher elevate him?
One might assume that all of the fallen are alive, since in Homecoming, the JW notes that, before Shiver's fall, the light has never challenged a fallen lord and won. When the JW accounts for the avatara, calling them "something like the three now," are the deaths of all of the other 6 mentioned? Could the unmentioned avatara have fallen previously in the war, when challenging one of the nameless fallen, this being why there's no mention?
