achievements are screwed up on destiny, maybe its a similar issue with Halo 5. try doing a power cycle. hold down the main power button until the console turns off (don't turn off with kinect or controller). unplug the power for a bit.

also try loging yourself out, and logging another gamertag in, then log yourself back in.
