They did mention earlier in the film that billionaires often made trips to Earth (Carlyle could obviously just go back and forth for a meeting), so that somewhat explains the mobile med-bays. As for why there could be a dozen onboard...

Maybe they did make occasional trips to Earth and charged thousands for a use. Have to leech off the population somehow, no? It calls to mind the production lines, and the tokens that Fink would give to his workers in Bioshock Infinite. Even at exorbitant prices, you can bet there'd be a line.
