It's based on wins and who you win against.
But what constitutes a 'win' differs by playlist:
In Team Ranked Playlists (Throwdown, Castle DLC, Team Objective) it is based on game outcomes - you win and lose as a team, against the other team.
In Individually Ranked Playlists (Infinity Slayer, Action Sack, SWAT) it is based on your personal score against everyone else in the game - ignore your team's result, acquire points.
The maths is based on TrueSkill - which is too complicated to explain here. But your rank will move more quickly to start with and then slower later on. In playlists with fewer, larger teams, it gets less information about you as an individual player per game. So it takes more games to change your rank.
Castle is 6v6, whereas Action Sack is effectively 8-man FFA.
