The following diagram shows this - if the shooter fires at the red line, you will die (and the bullet will appear to go through the cover). But if he fired at the blue line, you will survive. But in both cases you are not behind cover on the shooter's box.
Yes, the host allows for a small window of time after death where if the client says "I shot this guy" the gun will still fire. In many cases this is a headshot and both players will die.
